The Cox Maze III procedure has been the gold standard for the surgical treatment of atrial fibrillation. Radiofrequency and microwave energy have been used in recent years as an alternative to the “cut and sew” technique of the Cox-Maze-Procedure. Between January 2001 to October 2006, 104 patients underwent a modified Maze procedure with concomitant cardiac surgery at the Heart Center, Munich. The aim of this study was to examine the restoration of sinus rhythm. On the arrival at the intensive care unit, there were 79% in sinus rhythm, at discharge 56%. After 10-20 months 49% and after 21-48 months 39% were in sinus rhythm. The procedure was safe. There were no complications associated with the procedure. Duration of pre-operative atrial fibrillation, age, underlying disease and the left atrial diameter were determining factors for recurrent atrial fibrillation. Further studies are necessary to examine the effectiveness of the alternative procedures.
