In the Eurotransplant Senior Program (ESP) kidneys of elderly donors are locally allocated and transplanted without HLA-matching.
The analysis of 96 locally transplanted ESP-patients (receiver: 67a, donor: 72a) showed significant differences in the cold ischemia time (8,7h ESP vs.14,3h) with comparable primary organ function in a short-term comparison to a HLA-matched control group of the recent award scheme (n=46, receiver 64a, donor: 40a). In the long run, however, a significantly higher rate of rejection episodes (67% vs. 40%) was revealed by the ESP-group. This had bad qualitative and quantitative effects on the transplants, as, despite of equal patient survival, the graft loss was significantly higher in the ESP-group (five-year survival: 56% vs. 91%). This was particularly due to chronic graft loss, especially of donororgans above 75 years. These results suggest a necessity of HLA-matching also of the old organs.
