Im Rahmen des internen Qualitätsmanagements wurden 29 osteosynthetisch mit Kleinfragmentschrauben (KFS) über retroaurikuläre Zugänge versorgte Patienten mit insgesamt 36 ein- und beidseitigen dislozierten Gelenkwalzenfrakturen der Mandibula prospektiv nachuntersucht. Nach Materialentfernung wurden die funktionell-morphologischen Ergebnisse mittels Kernspintomographie, elektronischer Achsiographie und anhand klinischer Befunde erhoben. Es konnte objektiviert werden, dass die mit Positionierungsschrauben (Titan) versorgten Gelenkfrakturen biomechanisch stabil waren und die diskokondyläre Einheit anatomisch rekonstruiert wurde. Das atraumatische Design der KFS ermöglichte die Schonung des intra- und periartikulären Weichgewebes. Kondyläre Bewegungseinschränkungen werden auf Briden im Bereich der ehemaligen Bruchspalten zurückgeführt. OP-assoziierte Komplikationen konnten mit dem Wechsel des präaurikulären auf den retroaurikulären operativen Zugang reduziert werden.      «

In terms of quality control 29 patients with 36 uni- and bilateral displaced fractures of the condylar head and high condylar neck underwent a prospective assessment. All fractures were treated by ORIF with small fragment positional screws (KFS) via retroauricular approaches. After removal of OSM functional and morphological results were evaluated by magnetic resonance imaging, computerized axiography and clinical investigation. Titanium KFS showed good biomechanical stability in the spongious bone of the condylar head allowing anatomical reconstruction of both condylar head and the discoligamental structures and early functional training. The atraumatic design of the KFS prevented from scar formations of intra- and periarticular soft tissues. Limited condylar translations were shown to be mainly related to scar formation in regions of former fracture gaps. Replacing the preauricular by the retroauricular approach significantly reduced surgical complications.     «
