Kiefergelenkfortsatzfrakturen stellen hinsichtlich der Diagnose und Therapie die am kontroversesten diskutierte knöcherne Kontinuitätsunterbrechung des Gesichtsschädels dar. Von Januar 2000 bis Oktober 2009 wurden bei insgesamt 175 Patienten der Mund-Kiefer-Gesichtschirurgie Kollumfrakturen der Spiessl-Klassifikationen I - IV operativ behandelt, davon wiesen 57 postoperative Komplikationen auf. Mit 46% (37 von insgesamt 82 auftretenden Komplikationen) stellten Nervstörungen die häufigste Komplikation gefolgt von Fehlstellungen und Okklusionsstörungen mit jeweils 11% (9/82) dar. Innerhalb der gängigen operativen Zugangswege erwies sich der retromandibuläre Zugang als der Zugang der Wahl. Auch haben sich Miniplattenosteosynthesen aufgrund der niedrigen Dislokationsrate bewährt. Insgesamt sind sowohl die klinischen als auch die axiographisch erhobenen postoperativen Ergebnisse als positiv zu bewerten.     «

Concerning diagnosis and treatment mandibular condylar fractures represent the most controversially discussed fracture of the head. From January 2000 to October 2009, a total of 175 patients with condylar neck fractures according to Spiessl classifications I – IV have been treated surgically in the department oral and maxillofacial surgery, of which 57 showed postoperative long term complications. With 46% (37 out of 82 of complications), nerve disturbances were the most common complication followed by misalignment and occlusion with each 11% (9 / 82). Within all approaches the retro-mandibular access proved to be the access of choice. Also, due to the low dislocations rate mini-plate osteosynthesis have proven to be the most suitable. Overall, both clinical and axiographic results had to be evaluated as positive.      «
