Während der Behandlung von arteriellen Stenosen durch PTA mit/ohne Stent wird Heparin zur Antikoagulation eingesetzt. Dies senkt das Risiko thrombembolischer Komplikationen, erhöht aber das für Blutungen und Hämatome, was sich durch Protamingabe reduzieren ließe. Ziel der Arbeit war es, ein idealtypisches Dosierungsschema für die Antagonisierung von Heparin durch Protamin zu entwickeln. In einem Versuchsablauf wurde bei 60 Pat. die Wirkung von Heparin und Protamin mit der ACT verfolgt. Als wesentliches Ergebnis konnte aus dem erhobenen Datensatz eine Regressionsgleichung erstellt werden, die eine Berechnung der Protamindosis ermöglicht: Protamindosis = 2263 IE + 13 • Ist-Status – 19 • interindividuelle Interventionsdauer. Erstmals ist es mit dieser Gleichung möglich, ohne Messung während der Intervention die Protamindosis zur Antagonisierung des zuvor verabreichten Heparins (5000 IE) zu berechnen.     «

During the treatment of arterial stenosis with PTA with/without stenting, heparin is used for anticoagulation. With heparin, the risk of thrombembolic complications is scaled, but the risk for bleeding and hematoma is increased. These risks can be reduced with the application of protamine. Aim of this work was to discover an ideal application schedule for protamine. In an examination with 60 Pat, the effect of heparin and protamine was measured by the ACT. The basic result was a regression equation for the calculation of protamine dose: protamine dose = 2263 IE + 13 • Ist-Status – 19 • interindividuelle Interventionsdauer. For the first time, it is possible to calculate the protamine dose for the reversal of applicated heparin (5000 IE) without any measurement during the intervention.     «
