The aim of this trial was the impact of a neoadjuvant radiochemotherapy in locally advanced rectal cancer (uT3,4) on local recurrence and overall survival. Therefore 104 patients, who received neoadjuvant radiochemotherapy were compared to 114 patients (uT3,4), who were treated with a postoperative radiochemotherapy according to the NCI-protocol.
The neoadjuvant radiochemotherapy was well tolerated, only few complications occurred. The rates of complete resection and overall rate of postoperative complications did not differ between the groups, but a significant increase in sphincter preservation was achieved among patients who received neoadjuvant radiochemotherapy.
14% of the patients in the neoadjuvant treatment group had a complete response, a downsizing was achieved, a downstaging readily identifiable.
After complete resection the local recurrence occured significant less in the neoadjuvant treatment group, distant recurrence did not differ between the groups.
Local recurrence can be increased by a neoadjuvant radiochemotherapy. Neoadjuvant radiochemotherapy has a positive impact on overall survival.
