Aprotinin ist ein unspezifischer Proteaseninhibitor und wird in der Herzchirurgie aufgrund seiner blutungsverringernden Wirkung häufig verwendet. Als tierisches Eiweiß besitzt es antigene Eigenschaften und kann allergische Reaktionen auslösen. Translated abstract:
The non-specific proteinase inhibitor aprotinin is widely used in cardiac surgery to reduce bleeding. This protein has antigenic properties and hypersensitivity reactions may occur. The aim of this work is to evaluate the incidence of anaphylactic reactions to aprotinin in a large single center database. Of 13315 operations 12403 were done with aprotinin. Of this 12403 exposures 11602 were primary exposures and 801 were reexposures. 11 reactions to aprotinin were recorded after primary exposure (11/11602 = 0,1%), of which none was severe, while 12 reactions - 5 of them severe - occurred during reexposure (12/801 = 1,5%). There is a significant time dependent risk of allergic reactions during aprotininreexposure: respectively 4,1%, 1,9% and 0,4% in the < 6 months, 6-12 months and > 12 months reexposure intervals. Therefore aprotininreexposures during the first 6 months are strictly contraindicated. At a later date the drug can be given if the risk of bleeding is high and precautions are taken.
