In this study, a new noninvasive MRI-method for the diagnosis of patients with carotid artery stenosis and occlusion is presented. Based on the principle of arterial spin labeling, it allows for the combination of a dynamic spin labeling angiography with a particularly high temporal resolution (DSLA) with the measurement of cerebral perfusion. 21 controls, 30 patients with a carotid artery stenosis and 10 patients with a carotid artery occlusion were examined. It was verified, whether this technique is suitable for determining the degree of stenosis and the intracranial collateralization.10 of the patients with a carotid artery stenosis were re-examined after surgical or interventional treatment The results were statistically analyzed.The combination of DSLA and perfusion measurements was shown to be a promising method. It can identify patients at a hemodynamic risk, as well as monitor the hemodynamic effects of interventional vascular procedures.
