Die Behandlung von affektiven Erkrankungen und Erkrankungen aus dem schizophrenen Formenkreis wird in der Zukunft eine der größten Herausforderungen für das Gesundheitswesen darstellen. Die Pharmakotherapie stellt einen Eckpfeiler für die Behandlung der genannten psychiatrischen Erkrankungen dar. Allerdings sprechen etwa 30-50 % aller Patienten nicht oder nur ungenügend auf ihre primäre Psychopharmakotherapie an, oder leiden an zum Teil schwerwiegenden Nebenwirkungen. Es wird geschätzt, dass etwa 20-95 % der Variabilität der Arzneimittelwirkungen und Nebenwirkungen durch Variation in den Genen verursacht wird. Ziel der vorliegenden Arbeit war es, im Rahmen einer klinischen Studie die Assoziation zwischen 18 Polymorphismen in verschiedenen Kandidaten-Genen und der Effektivität bzw. Sicherheit einer Psychopharmakotherapie zu untersuchen. Zum Nachweis der Polymorphismen wurden neben konventionellen Genotypisierungsmethoden auf Fluoreszenz basierende „real time“ Methoden entwickelt. Für insgesamt 3 Polymorphismen in den untersuchten Kandidaten-Genen konnte eine signifikante Korrelation zwischen Genotyp und klinischem Erfolg der Therapie gezeigt werden (Serotonintransporter [HTT]LPR/HTT-VNTR Polymorphismus und Nebenwirkungen HTT-blockierender Antidepressiva sowie Dopamin D4 Rezeptor [DRD4] 48 bp Wiederholungs-Polymorphismus und Gewichtszunahme unter der Therapie mit Antipsychotika). Nach Bestätigung der Ergebnisse in weiterführenden Studien könnte dies möglicherweise ein erster Schritt auf dem Weg zu einer individuell angepassten Psychopharmakotherapie auf der Basis einer vorherigen Genotypisierung sein.     «

The treatment of affective and schizophrenic disorders will be one of the biggest challenges for the health care system in the future. The pharmacotherapy is a cornerstone for the treatment of the mentioned psychiatric illnesses. Indeed, about 30-50% of all patients do not or only insufficiently respond to their primary psychopharmacotherapy, or suffer from partly serious side effects. It is estimated that about 20-95% of the variability of the effects and side effects of medicinal products are caused by genetic variations. The purpose of the presented dissertation was to investigate the association between 18 polymorphisms in different candidate genes and the efficacy or safety of psychopharmacotherapy within the scope of a clinical study. For detection of the polymorphisms conventional genotyping methods as well as fluorescence-based "real time" methods were developed. For a total of 3 polymorphisms of the investigated candidate genes a significant correlation between genotype and clinical success of the therapy could be shown (serotonin transporter [HTT]LPR/HTT-VNTR polymorphism and side effects of HTT blocking antidepressants as well as dopamine D4 receptor [DRD4] 48 bp repeat polymorphism and increase in bodyweight caused by antipsychotics).
