Eine mögliche Folgeerkrankung nach Schädel-Hirn-Trauma ist eine posttraumatische Unterfunktion der Hypophyse. In dieser Arbeit wurde in einem experimentellen Mausmodell untersucht, ob posttraumatischer Hypopituitarismus durch eine Schädigung des Hypothalamus verursacht werden kann. Es konnte gezeigt werden, dass das Trauma zu einer signifikanten Reduktion der Nervenzellen des Hypothalamus führt, die im zeitlichen Verlauf progredient ist. Als mögliche Ursache dafür zeigten sich diffuse Axonschäden. Neben diesen direkten Schäden konnte ebenfalls die Ausbildung einer Gliose als indirekter Marker einer Hirnparenchymverletzung nachgewiesen werden. Die Ergebnisse deuten also darauf hin, dass mikroskopische Schäden des Hypothalamus eine Rolle in der Pathogenese der Erkrankung spielen.      «

Hypopituitarism is known to be a major complication of traumatic brain injury- even in patients without any macroscopic damage to the pituitary. The aim of the present study was the examination of morphological changes of the hypothalamus in an experimental mouse model of TBI. Both 15 and 30 days after injury, a significant reduction of neurons could be observed, most probably caused by diffuse axonal injury. In addition, reactive changes including a significant gliosis and an intense activation of microglial cells in the hypothalamus could be demonstrated. Consequently, the data indicate traumatic changes in the hypothalamus as a potential cause in the pathogenesis of hypopituitarism after TBI.     «
