239 Patienten mit V.a. Choledocholithiasis wurden mittels initialer Endosonographie (EUS) untersucht. Bei Steinnachweis (32,9%) erfolgte die therapeutische ERCP. Bei Steinausschluß, wurde auf eine ERCP verzichtet es erfolgte eine Nachbeobachtung. Die EUS zeigte eine hohe diagnostische Genauigkeit bezüglich einer Choledocholithiasis (Sensitivität 89,7%; Spezifität 100%). Es kam zu keinen EUS assoziierten Komplikationen. 129 ERCPs und damit 6,3 assoziierte Komplikationen konnten vermieden werden. Im Vergleich zu der bisher üblichen ERCP Diagnostik ergab die EUS gesteuerte Diagnostik gleiche Erlöse bei geringeren Kosten. Daher ist der EUS basierte Algorithmus aus klinischen und ökonomischen Gründen zu empfehlen.     «

In the absence of stones, no ERCP was performed, the patients were followed. The EUS showed a high diagnostic accuracy concerning a choledocholithiasis (sensitivity 89,7%; 100% specificity). There were no EUS associated complications. 129 ERCPs and therefore 6,3 complications could be avoided. Compared with the conventional diagnostic ERCP the EUS guided diagnostic resulted same benefits with lower costs. The EUS based algorithm is recommended because of clinical and economic reasons.      «
