Die Spontan Bakterielle Peritonitis (SBP) ist die häufigste bakterielle Infektion bei Patienten mit zirrhotischem Aszites. Die vorliegende retrospektive Untersuchung wurde für Januar 2002 bis August 2006 an 101 stationären SBP-Fällen des Klinikum rechts der Isar durchgeführt. Sie analysiert das Resistenzspektrum nachgewiesener Erreger, die Häufigkeit von Therapieversagern unter leitliniengerecher Antibiose und deren Einfluss auf die Mortalität. Fast ein Viertel der Erreger war resistent auf die initiale Therapie. In dieser Gruppe war die Mortalität erhöht. Die Arbeit erhebt den wichtigen Befund eines Wandels des Erregerspektrums der SBP und wirft die Frage nach therapeutischen Konsequenzen auf.     «

Spontaneous Bacterial Peritonitis (SBP) is the most common bacterial infection in cirrhotic patients with ascites. This retrospective study examined 101 inpatient cases of SBP of Klinikum rechts der Isar from January 2002 to August 2006. It analyzes resistance spectrum of cultured pathogens, frequency of treatment failure of guideline antbiotic therapy and their impact on mortality. Almost a quarter of the pathogens were resistant to first line therapy. In this group, mortality was increased. This study shows the important finding of a change in spectrum of pathogens of SBP and brings up the question of therapeutic consequences.     «
